Dear Parents and Caregivers, 

1. To enroll your child and reserve his/her space in program, I need:

• Enrollment Application (signed and dated) 
• A non-refundable $50 registration fee and non-refundable $200 tuition deposit (secures the program space for your child; applies to First Monthly Payment). Please make CHECKS payable to ANASTASIYA TARNAVSKY. 

2. Before/on your child’s first day of attendance, I will need:

• Filled out Family Child Care Enrollment Packet and Photo Release Permission Slip
• Recent physical examination report showing immunizations and a lead test.
